In the quest for effective weight loss solutions, GLP-1 medications like Ozempic, Wegovy (both semaglutide), and Zepbound (tirzepatide) have emerged as promising options. These drugs not only help control appetite but also improve metabolic functions, offering a multifaceted approach to help with weight management.  However, the soaring demand and too frequent shortages of these name-brand medications led the FDA to place these medications on their ‘drug shortage list’.  While on the drug shortage list it is legal for other companies to manufacture the drugs despite the patent on the medication.  This has led some individuals to seek alternatives often found on-line from clinicians who prescribe compounded GLP-1s and compounding pharmacies which manufacture them.

While this might appear to be a convenient or cost-effective shortcut, sourcing GLP-1 medicines is just a temporary option. Zepbound was removed from the drug shortage list on October 2nd, and as of October 30th Wegovy/Ozempic are listed as ‘available’ by the FDA; it is anticipated that they will be removed from the drug shortage list very soon.

But more important to you than issues related to a patent is that taking compounded GLP-1 medications carries significant risks that could jeopardize both your health and your weight loss goals.

The Rise of Compounding Pharmacies for GLP-1 Medications

The high demand for GLP-1 medications has sometimes outpaced supply, prompting some to turn to compounding pharmacies. These pharmacies traditionally customize medications to meet specific patient needs but have recently begun offering GLP-1 medications, authorized by the FDA, only during times of low market supply. It’s important to recognize that this is a temporary situation driven by market dynamics and not a long-term solution.

Being placed on the FDA’s drug shortage list has prompted an explosion of compounding pharmacies offering GLP-1s. When it comes to online compounding pharmacies, the National Association of Boards of Pharmacy, has found more than 40,000 online pharmacies operating outside of their recommended guidelines and sometimes even illegally. “There are legitimate pharmacies that operate online and I would say probably 5% of all pharmacies that operate online are legitimate,” says Al Carter, NABP’s executive director. Even if a patient is not obtaining GLP-1s directly from an on-line compounding pharmacy, the question remains as to where the patient’s direct supplier obtained the medication.

As a result, the FDA has recommended against the use of compounded GLP-1 medications, despite making it legal to sell them.

GLP-1 medications can cause side effects ranging from mild nausea to severe complications like pancreatitis or kidney problems. Effective weight loss is not achieved just by getting a GLP-1 prescription. Without support in areas like nutrition and physical activity, you may miss out on the full benefits of GLP-1 therapy and are more likely to experience weight regain once the medication is discontinued.

5. Untrustworthy Online Sources of Medicine

If you’re considering buying semaglutide online, it’s crucial to ensure the website you’re using is trustworthy. Many new online services that offer medication delivery provide compounded versions of popular drugs, which are made either in-house or through partnered compounding pharmacies. However, be extremely cautious: some companies selling compounded semaglutide may not comply with state and federal regulations. Fake versions of medications like Ozempic are also being sold in the U.S., potentially containing harmful or incorrect ingredients and some are mislabeled as “for research purposes” or “not for human consumption” but include dosing instructions.

Novo Nordisk, the manufacturer of brand-name semaglutide, has taken legal action against several businesses for selling inferior compounded versions of the drug. They have discovered issues such as impurities, incorrect drug concentrations, and even products containing no semaglutide at all. Some compounded products use different forms of semaglutide, like semaglutide sodium or semaglutide acetate, which are not the same as the approved active ingredient. Their safety is unknown. According to a company statement, there have been hundreds of reports submitted to the FDA’s voluntary adverse event reporting system related to compounded semaglutide.

So it is crucial to know the quality and safety of the compounded GLP-1 medication you are considering, regardless of wherever or from whomever you would obtain it.  But short of analyzing a sample, how are you going to do this ??  And even most very well-intentioned, trusted suppliers do not have the ability to analyze the product they are dispensing.

6. Unreliable Supply and Market Instability

The availability of compounded GLP-1 medications is uncertain and subject to change without notice. As pharmaceutical companies increase the production of name-brand medications, compounding pharmacies will have to cease their offerings of GLP-1 drugs. This unpredictability can disrupt your treatment plan, forcing abrupt changes that may hinder your progress and affect your health.

Many insurance providers require patients to participate in structured weight loss programs to qualify for coverage of GLP-1 medications. These are prescription medications, but obtaining them without first obtaining a medical evaluation is possible and frankly foolish for the multiple listed reasons.

The next decision is your selection of a health care provider. It is increasingly recognized that GLP-1 medications are much more effective if used as part of a care program provided by teams experienced in specialized weight management. In fact, some insurance companies have started to approve coverage for GLP-1 medication only when prescribed by a list of approved providers. This is because many primary care practices and the numerous commercial programs offering GLP-1s that have sprouted up in the past 12 months cannot offer the experienced teams of providers necessary to obtain the best weight loss results.
